A Review on the Use of Tumescent Solution in Endovenous Thermal Ablation for Saphenous Vein

Abstract
Purpose: The use of a tumescent solution in endovenous thermal ablation has many advantages. There is an analgesic effect by lidocaine itself. It expands the surrounding tissue of saphenous vein, protects the surrounding tissues from the heat, presses the vein, empties the blood, contracts the blood vessels and brings the catheter into contact with the blood vessel wall. However, the use of thermal ablation has the disadvantage of causing pain due to multiple skin punctures. Overuse of tumescent can occur lidocaine toxicity. The purpose of this study is to investigate the use of a safe and effective tumescent solution.

Methods: Reports related to the tumescent solution for saphenous thermal ablation reported to date were investigated.

Results: Multiple injections of a tumescent solution can cause pain in and of itself. Large amount of expanded anesthetic solution can lead to lidocaine side effects. Tumescent solution administered perivenously have been reported to be safe up to 15 mg/kg according to the literatures.

Conclusion: It is necessary to calculate the dose in advance for a large volume of tumescent solution or for very dry patient and to observe the symptoms for at least 12 hours if the maximum dose is used. In addition, we have to design a puncture method suitable for the practitioner so that we can inject a sufficient amount with a minimun of puncture.
